Your Rights

We will:

• Provide treatment that is safe, effective and based on the best evidence available
• Communicate effectively and sensitively with patients and their advocates
• Listen to the concerns and views of patients and investigate any complaints made via the complaints procedure
• Treat patients with dignity and respect regardless of culture, race and beliefs
• Involve and inform patients in decisions about their care and treatment
• Respect patients' rights to privacy and confidentiality
• Regularly review patients with chronic illnesses
• Regularly review patients on repeat prescriptions
• Screen for risk factors for heart disease, stroke and diabetes
• Provide advice on healthy living

Your Responsibilities

We ask that you:

• Treat all practice staff politely and with respect. We have a zero-tolerance policy regarding any abusive or violent behaviour or any racial, sexual or verbal abuse. Patients who do not follow these requests will be removed from the practice by the police if necessary and removed from the practice list.
• Follow the advice and treatment you are given. If you are worried about doing this please discuss your concerns with the health professional giving you the advice
• Make sure we have up to date information on your contact details - address, telephone number, etc.
• Be on time for your appointment and cancel if you are not going to attend
• Look after your own health and make decisions to promote a healthy lifestyle.
